Delta Dental Plan
The ACCMA and Delta Dental are pleased to offer a NEW HMO plan into our member portfolio for 2008 open enrollment. As always the plans are available to all ACCMA members (active or retired) and family, on a guaranteed issued basis. The Dental plans are available to ACCMA member employees too. Please submit your ACCMA Delta Dental application to JC Lewis Insurance Services to process your enrollment promptly. The application and coverage change request forms are listed below for you to print out, fill out and send back for enrollment.

When to apply for ACCMA Delta Dental plans?
The ACCMA open enrollment for Delta Dental plans begin December 1, 2008 and runs thru the end of December 2008. The effective starting dates will be January 1st, 2008. However, you may apply for dental plans after the open enrollment and rates will be pro-rated.
How do you get billed for ACCMA Delta Dental plan?
All the billing and collection for ACCMA Delta Dental plans is done by JC Lewis Insurance Services. After completing the application process you will be added to the ACCMA Delta Dental billing system and you should receive a semi-annual bill. Groups may elect monthly billings.
