California Individual Health Insurance
If you are under the age of 64 and don't have coverage through an employer or union you may enroll in a California Individual Health Insurance Plan.
Your acceptance into an Individual or Family Plan (IFP) is not guaranteed. Therefore, you may be approved, approved at an increased premium or declined coverage. To apply for an IFP you will be required to complete an application with numerous health questions. Here are several categories which if prepared in advance will assist you in providing a complete application:
- Your height and weight
- List any prescription medication, dosages and frequency
- Do you have any health issues
- Have you been recommended to have any future diagnostic testing or treatment
- List your last doctor visit with full details of the visit as well as your doctors name, address and phone number
